Evania Healthcare Clinic
Prescription Renewals, Letters and Forms.
Whenever possible, your prescriptions are dispensed to last until your next scheduled assessment. While some medications may include additional refills, please confirm this with your pharmacy. If no refills remain, it is likely due to the need for medical monitoring or reassessment. It is your responsibility to keep track of your medications and book renewal appointments 2–3 weeks in advance, as urgent appointments for regular medication renewals cannot always be accommodated.
For prescription refills, please ask your pharmacy to send us a renewal request. We are not currently charging for refill requests, but fees will be introduced in the future. The best approach is to bring your medications to your visit so prescriptions can be renewed during your appointment and last until your next scheduled follow-up.
For forms (e.g., sick notes, letters for physiotherapy, massage, orthotics, or insurance documents), we use a secure platform called DocNote. Please register at docnote.com, upload your form, and provide any relevant information via messaging. The physician will review and advise if an in-person visit is needed. As forms are not covered by OHIP, there will be a fee, which is posted on DocNote and on our website under Uninsured Services. For questions about the platform, please contact DocNote directly.

Use Of Walk In Clinics
Evania Healthcare Clinic operates as a Family Health Organization (FHO), allowing us to provide timely, personalized care and high-quality services to our patients. All of our physicians reserve a few same-day appointment slots each day for urgent concerns. In addition, our after-hours clinic is available for urgent care from 5:00 PM to 8:00 PM on Tuesday to Thursday, and from 9:00 AM to 2:00 PM on Saturday.
Our clinic receives funding from the Ministry of Health based on the understanding that enrolled patients will seek care through Evania Healthcare Clinic rather than walk-in clinics whenever possible. We do not receive medical records from walk-in clinics, which can compromise the continuity and quality of your care. This can also lead to unnecessary duplication of tests or treatments. In addition, the use of walk-in clinic services may result in a financial penalty for your doctor and the clinic.
We understand that this model of care may not meet everyone’s needs, and some patients may prefer to access walk-in clinics. If this applies to you, please speak with your doctor so we can determine the best approach for your care.

Appointments
When you first register with our clinic, you will be offered a meet-and-greet appointment with one of the physicians currently accepting new patients. Due to high demand and long waitlists, we are only able to offer one introductory appointment per patient—you will not be able to meet with multiple physicians before choosing.
Once you have established care with a physician, we do not permit switching to another doctor within our practice.
For urgent same-day concerns, you may be seen by another physician from our team if your regular doctor is unavailable. However, routine appointments should be booked with your own family physician to maintain continuity of care.
To help us schedule your visit appropriately, please inform our administrative staff of the reason for your appointment when booking. Most standard appointments are scheduled for 10–15 minutes. If you believe you may need additional time, please let us know in advance.


Cancellation/Missed Appointment
We require cancellations to be made at least two business days prior to your appointment. This allows your appointment slot to become available for other patients who require care. If you miss an appointment or cancel with insufficient notice or reason, you will be billed the cost of the visit.
Please arrive on time for your appointment.
We value your time and will strive to see you at your scheduled appointment time. If you are late for your appointment, we may ask you to wait until the last patient has been seen, or you may be asked to reschedule.

Safe Space
We respect our patients of all
backgrounds, religions, races, genders, sexualities, and beliefs. It is our intention to create a safe space and welcoming environment for all of our patients and staff.
We ask that everyone attending the clinic behave in a respectful manner towards staff and other patients at all times. If a behavior is deemed to be at odds with this policy, we reserve the right to recommend a patient seek care elsewhere.

Uninsured Services
Some services, including sick notes, forms, driver's medicals, are not covered by OHIP. You may be charged for them according to the rates recommended by the OMA. Please see the list of uninsured services available at our reception.
We will do our best to complete your forms in a timely manner. However, please allow 5-10 business days for any form to be completed.
